Advantages of the Pterional Approach for Anterior Skull Base Meningiomas

Abstract
Objective: The authors report on our experiences of pterional approach for olfactory groove meningioma(OGM), tuberculum sellae meningioma(TSM) and planum sphenoidale meningioma(PSM) in 7 patients.

Methods: Between February 2002 and January 2009, we performed macroscopic tumor removal via pterional approach in 7 patients(6 female, 1 male) with OGM, TSM or PSM. The mean longest diameter of mass was 39.9¡¾11.7 mm and duration of symptom was 24.6¡¾24.8 months. The most common symptom presented by the patients was visual impairment( 57.2%) followed by headache, seizure and anosmia(each 42.8%). And three patients presented also with personality change. The authors retrospectively reviewed patients¡¯ clinical and radiological data.

Results: In all 7 patients, total removal of tumor(Simpson grade I) was performed and there was no regrowth during the follow-up period(median£»38.3¡¾29.1 months). And opthalmological or other neurological complications did not occur. Only 1 patient suffered from sepsis due to pseudomembraneous colitis.

Conclusion: The authors are of the opinion that compared to other approaches for anterior skull base meningiomas, the pterional approach is a superior approach.
